Irvine Residents Ask: Is Aesthetic Hand Rejuvenation for Me?

Cosmetic surgery has become a very popular and accepted fixture in our society today with a myriad of techniques and products available to help rejuvenate and enhance the face and body. The face is the #1 most exposed portion of our bodies that people immediately recognize and observe, and as a result, there is a large spectrum of procedures aimed to maximize its aesthetic appeal, from the less invasive neurotoxins, soft tissue fillers, chemical peels, and lasers, all the way up to the more aggressive and effective surgical facelifts. However, the second most commonly exposed region of our bodies that most people overlook are our hands.

Your Hands May Give Away Your Age

Although patients can spend thousands of dollars trying to rejuvenate their face, I cannot tell you the number of patients who, despite having a wonderful facial outcome, their hands give away their age immediately. Studies have been published in the scientific journal, Plastic and Reconstructive Surgery®, concluding that just based on examination of the hands, most people can accurately guesstimate a person’s age. Therefore, in order to obtain a balanced aesthetic outcome after facial or body rejuvenation, it is recommended to also treat the hands, especially if you have advanced signs of aging that may display your true age.

As a brief overview, there are several well-defined changes that occur to the hands due to aging. If you examine your child’s or grandchild’s hands the next time you see them, you will notice a youthful fullness to the back of their hands with smooth, unblemished, elastic skin, and a lack of prominent veins or wrinkles. As we age, there is a gradual thinning of the dermal layer of the skin with formation of fine wrinkles, age spots, and even skin cancers. There is also gradual atrophy of the subcutaneous fat layer and intrinsic muscles with the prominence of underlying veins, tendons, and joints. In addition, you can develop bony deformities from the occurrence of arthritis or other pathologies. These signs of aging can be exacerbated by factors such as smoking, excessive sun exposure, and lifestyle or occupational insults to the hands. Our hands are critically important for our daily activities, both functionally and visually.

What are Your Hand Treatment Options?

Treatments are available to address each of these concerns, many of which require minimal downtime.  Dermal thinning and superficial skin dyschromias can be managed with a variety of products. Retin-A, ie. tretinoin, is the only cream proven to thicken the dermis and help remove the outer damaged skin layers. Glycolic acid, Jessner’s solution, and trichloroacetic acid (TCA) peels are just a few of the chemical peels available to help exfoliate and rejuvenate the hands. Microdermabrasion performed in the office setting helps smooth the outer skin layer, but will require several treatments for maximal effect. A number of lasers are also available, the most effective of which is the fractional CO2 laser, which can be performed in the office, but does require some post-treatment care and downtime for a few days.

Dorsal hand volume loss can be addressed with volumizing treatments, including augmentation with autologous natural fat grafting, that is typically done in the operating room, or more recently, with soft tissue fillers in the office setting. Prominent veins can also be managed by direct excision (phlebectomy) or by vein coagulation (sclerotherapy). Finally, any deformities that may be present as a result of trauma, osteoarthritis, ganglion cysts, Dupuytren’s contracture, or any of a number of other pathologies can be improved with hand surgery.

Irvine Residents Ask: What is a Mommy Makeover?

Mommy makeovers are popular cosmetic procedures performed today. The term implies aesthetic surgery for improvement of certain changes that mothers endure with pregnancy. However, it can be applied more broadly to other patients as well, including excessive weight loss patients with severe skin excess from dieting or gastric bypass surgery. Essentially, a mommy makeover is any combination of cosmetic procedures aimed to rejuvenate various aspects of the body that have undergone the ravages and stresses of pregnancy, aging, and large fluctuations in weight. It is important to consult with a fully-trained and knowledgeable plastic and reconstructive surgeon to determine which procedures are appropriate to address and improve each problem area.

There are many characteristic changes that occur with each stress that can lead to an unaesthetic appearance and can negatively affect self-esteem. The large expansion of the abdominal wall with pregnancy often leads to significant skin laxity and spreading of the rectus abdominis muscles (ie. Diastasis recti) with stretch marks that cannot be improved upon with exercise alone. For this problem, an abdominoplasty or “tummy tuck” would best address the removal of excess tissue, including many of the stretch marks, with tightening of the rectus muscles. The engorgement of the breasts with milk followed by its subsequent deflation can lead to sagging, deflated breasts. They can respond nicely to a breast lift and/or breast augmentation surgery. Persistent fat accumulation after pregnancy, especially in certain problem areas such as the flanks (“love handles”), thighs, and inner arms will respond well to liposuction. These issues are commonly seen with the massive weight loss patients, but usually to a more severe extent that may require a full body lift, arm lift, or thigh lift to maximize aesthetic body contouring.

The aging process compounds and exacerbates all of the above changes. With age, our skin gets thinner, the elasticity of our skin goes down, and there is progressive sagging and deflation of our tissues. This is most notable not only on the body but on our faces. Deepening smile creases (nasolabial folds), tear trough creases (nasojugal folds), and marionette lines can respond to soft tissue fillers, such as Juvederm®, Restylane®, and Radiesse®, which is performed in an office setting. More severe lost volume can respond to natural fat grafting procedures. Hyperactive wrinkles of the forehead, glabella (number ‘11’ sign), and crow’s feet can be managed with Botox®, Xeomin® or Dysport®. Facial dyschromias and sun damage spots, which are very common here in sunny southern California, can be managed with chemical peels or CO2 laser resurfacing and a good skin care regimen. More severe sagging can be treated with a forehead, face, and neck lift surgeries. Eyelid laxity can also be addressed with a blepharoplasty, or eyelid lift surgery.

There are many rejuvenation options available to help mothers deal with the stresses and changes brought about by pregnancy and the aging process.  It is important to have a thorough consultation and candid discussion with a reputable and experienced plastic and reconstructive surgeon. Three Facelift Facts Newport Beach Residents Should Know

Hello, it’s Dr.Go from Newport Plastic Surgery with another GoPost!  For today’s topic, I will be discussing three interesting facts about facelift surgery that you should know.  Although facelifts are commonplace occurrences in our entertainment-centric pop culture, there is still an elusive air to what they really entail.  To help clarify some misconceptions, here we go:

Not All Facelifts Are Created Equal

Like with all things in cosmetic surgery, there is a certain artistic flair mixed with surgical technique that goes into every procedure, and not all facelifts are created equal.  In fact, if you put a dozen plastic surgeons in a room, it is safe to say that no two methods will be exactly the same. Even with several well-known and studied techniques in the plastic surgery literature, there will be variations of each procedure based on where the surgeon trained, which technique is most comfortable and reproducible to them, and adjustments in each case based on their own experiences of what worked and what did not work well previously.  As such, it should go without saying that it is important to go to a board-certified plastic surgeon who has had good experience performing facelifts to ensure the best possible outcome for you.

Results Are Not Permanent

It is true that certain facelift techniques lead to longer lasting and natural results, such as methods where deeper layers are lifted VS a skin-only lift that tend to lead to poor scars and the stretched-out ‘fake’ look.  However, some patients believe that once they have cosmetic surgery, the results will be permanent. This unfortunately is not the case and I try to educate my patients as such. Although cosmetic procedures can rejuvenate the face and body into a more youthful appearance, essentially setting back the clock, the aging process goes on due to the continual insults of time, sun damage, hormonal changes, and the natural progression of aging with thinning of the skin dermis, loss of elastin fibers, and stretching of supportive structures in our bodies.  It is important to understand that although patients will always look better than if they did not have anything done, the aging process will continue for which they may benefit from a touch-up procedure in the future.

It Is Not As Scary As You May Think

Many people are cautious or afraid of undergoing facelift surgery, especially after seeing the well-popularized poor results and complications of certain celebrities in the media, including Kenny Rogers, Joan Rivers, Meg Ryan, and Mickey Rourke.  There are also television shows such as Botched on the E!@ network (https://www.eonline.com/shows/botched) that showcases cosmetic surgery disasters that some patients have suffered.  That being said, facelifts are still a very safe procedure that can give amazing results and take years off the facial aging appearance as long as patients do their due diligence and find a board-certified plastic surgeon with experience to perform their procedures.  I personally have performed hundreds of facelifts, with the vast majority done in the comforts of my office under only local anesthesia, with great results. Not every patient is a candidate for local anesthetic alone, but with the proper workup and thorough discussion with your plastic surgeon, the procedure is overall safe and well-tolerated with minimal risks and complication rates.

Three Things Your Newport Beach Plastic Surgeon Wished You Knew

Hello, it’s Dr.Go from Newport Plastic Surgery with another GoPost!  Today I would like to discuss a few things that I think would be helpful for patients to know in order to maximize the doctor-patient relationship and to achieve the best results as a team.  I have had the privilege of taking care of hundreds of patients over the past 10 years in practice, and there are some recurring problematic themes that I see some patients display that I would like to shed some light on for everyone’s benefit, so here we go:

Working Together for Best Outcomes

Although a large number of my patients do a good job following my pre and postop instructions, I cannot count the number of times some patients do detrimental things to hinder their outcomes, then come back wondering why they did not get perfect results.  Some examples include patients who continue smoking during and/or after their surgeries leading to poor healing and bad scarring, resume an unhealthy lifestyle and bad diet after their body contouring procedures causing reaccumulation of fatty deposits, going back to work or working out too soon after their surgeries leading to hematomas, avoiding optimal skin care and sun protection after their skin resurfacing procedure causing the results to not last as long, going on vacation and swimming in dirty pools, lakes, or ocean waters too soon after their surgeries leading to wound infections, etc.  Although it sounds like common sense, it should go without saying that the best outcomes and results occur when you work together with your plastic surgeon and follow all of their pre and postop recommendations.

Realistic Expectations

It is important for patients to have realistic expectations.  I continue to be surprised and perplexed when I see a new patient for a consultation and they bring in pictures of supermodels and expect to look like them after their aesthetic procedure.  I have a saying that I like to use, which is that ‘I am a physician, not a magician’.  Every person is unique with their own body type, height, weight, medical history, list of medications that they are on, lifestyle habits including diet, exercise, and alcohol/ tobacco use, etc.  Thus, I need to explain that it is nearly impossible to obtain perfect results and achieve supermodel or celebrity proportions unless they possess very favorable characteristics to begin with.  Otherwise it is important to know that we will work together to achieve cosmetic improvements to their areas of concern and get as close as possible to their goals.  I have found that patients are happier when I am honest about what can realistically be achieved by managing their expectations.

Avoid Price Shopping

Everyone loves a good bargain, right?  This concept may be applicable when you are shopping for food, clothes, or even a new car where the actual items are a set commodity.  However, I would highly caution against using this method when you are looking into cosmetic surgery.  Searching for a legitimate surgeon to operate on you is the last place you want to bargain shop and gamble on your life.  Nevertheless, I still see a fair number of new consultations with patients who are price shopping for the cheapest possible rates to book their surgeries, and I always shudder when they decide to go elsewhere and risk their results and lives to an unqualified practitioner, sometimes overseas.  Cosmetic surgery is not a commodity where patients should just book with the lowest cost provider.  No two medical offices are the same and it is up to the patient to do their due diligence in researching their physicians and making sure that they are a board-certified plastic surgeon with good experience before proceeding with their surgery.

Liposuction vs. Fat Freezing aka CoolSculpting in Newport Beach, CA

Hello, it’s Dr.Go from Newport Plastic Surgery with another GoPost!  With our culture’s ongoing obsession with sucking out and removing excess fat from our bodies, there are always new inventions and advancements in technology that produce, or at least promote, that they are the latest and greatest method available for taking care of problematic fat collections in our bodies.  Today I would like to discuss one of these newer fat reduction trends backed by a strong marketing engine, CoolSculpting@ (https://www.coolsculpting.com/), and compare it with the tried and true liposuction procedure.

Traditional Liposuction

Suction-assisted lipectomy, ie. Liposuction (https://www.newportplasticsurgery.com/liposuction-irvine/), is the well-known and popularized method of fat removal that has been the subject of a number of television shows and movies.  Originally invented in the mid-1970s by Dr.Fischer of Italy, it has gone through many technological advancements and instrumentation updates over the years.  These revisions have led up to the current methods of liposuction, which is a safe and effectual way to remove excess fatty deposits in our bodies.  The procedure involves making small access incisions to infiltrate the subcutaneous space with tumescent fluid, which is typically a mixture of a numbing medication, such as lidocaine, with epinephrine, which leads to vasoconstriction and minimizes any blood loss.  Next, a series of thin cannulas are used to suction out the fatty deposits.  The traditional method requires the surgeon to move the cannula back and forth to facilitate this.  However, some of the newer iterations for liposuction have included power-assisted, where the handpiece vibrates and moves the cannula to help break up the fat more easily, ultrasound-assisted, where a probe with ultrasound heating is used to liquefy fat cells before suctioning, and laser-assisted, where a laser probe is used to deliver laser energy to destroy fat cells prior to their suctioning.  There is a vast array of cannulas that have been invented as well as techniques for micro-body sculpting and abdominal etching that have been used to maximize the results.  Liposuction can be performed with local or general anesthesia.

Fat Freezing (CoolSculpting@)

Cryolipolysis, ie. fat freezing, is a newer method of fat reduction that was researched and published initially by Dr.Manstein and Dr.Anderson of the US in 2008.  The technology was refined and packaged into the CoolSculpting@ platform by Zeltiq Aesthetics, Inc. an affiliate of cosmetic giant Allergan@.  The science behind this method involves using noninvasive vacuum-assisted applicators that are applied over fatty problem areas to extract energy and cool the underlying areas.  Fat cells, which are more vulnerable than other cell types to damage from this cooling mechanism, are induced into apoptosis, which is cell death.  Over the next several months, inflammatory cells gradually break down and absorb the remains of the destroyed fat cells, leading to reduced fat collections.  Although the treatment is noninvasive leading to faster recovery time, the results are typically much more subtle than a traditional liposuction procedure.  As such, sometimes multiple treatments are recommended, for which each session can last 1-2+ hours depending on the number of locations for treatment, as well as whether more than one machine is available for use.  The growing popularity of CoolSculpting@ can in large part be attributable to a major marketing campaign put on by its corporation with social media posts and television commercials leading to centers popping up on almost every corner.  However, it is important to note that some of these centers operate without any medical supervision, and total costs could end up being comparable or even more expensive than liposuction treatments.

The Verdict

So which method is better?  I believe that there are roles and indications for each technique.  If a person is terrified of any needles or the thought of having a thin cannula, no matter how small, be used to suction out their fat, then cryolipolysis may be the right choice for them, as long as they are aware that the results are typically subtle, require several months to see, may require multiple treatments, and may end up costing more in the long run.  Otherwise, for more noticeable, instantaneous results in fat reduction, I would recommend the tried and true liposuction procedure that has a proven track record of being a safe and effective method for maximally contouring the body.  If you would like to learn more about these fat reduction procedures, please give my office a call for a free consultation, and be sure to check out all of the helpful information on my website (https://www.newportplasticsurgery.com/).  Until next time, be well and stay tuned for my next GoPost!

The Top 5 Most Common Plastic Surgeries and What Orange County Residents Should Know

Hello, it’s Dr.Go from Newport Plastic Surgery with another GoPost!  With the United States again being the #1 society internationally to perform plastic surgeries, it is no wonder that other countries look to us as the pioneers and leaders of the cosmetic movement.  Today I would like to share with you the latest 2018 national cosmetic procedural statistics from the American Society of Plastic Surgeons (ASPS) which indicates another healthy year of growth with nearly 17.7 million cosmetic procedures performed, a 2% increase from the previous year.  Of these, 1.8 million are surgeries whereas the other 15.9 million are attributable to the minimally invasive procedures such as Botox@, which remain extremely popular.

Top 5 Cosmetic Surgeries

Topping the list of cosmetic surgeries once again is breast augmentation at 313,735 procedures performed in 2018, which is an increase of 4% over the prior year.  Also on the rise which comes in at #2 is liposuction with 258,558 cases performed, a 5% increase from the previous year.  Still popular with coming in at #3, but showing a 2% decrease from before, is rhinoplasty, with 213,780 surgeries performed.  Next comes eyelid surgery/ blepharoplasty at #4 with 206,529 cases, a 1% decrease from before.  Rounding out the top 5 is abdominoplasty/ tummy tuck at 130,081 surgeries performed, which is roughly unchanged from the previous year.  I would say that these statistics pretty accurately reflects my own practice, with the exception that rhinoplasty is replaced by face and neck lifts, which is much more popular in my practice.  I have noticed that rhinoplasties have been on the decline, at least in southern California, perhaps due to an emphasis on the other cosmetic surgeries on the list in our sunny, beachy climate.

Top 5 Minimally Invasive Cosmetic Procedures

As popular as cosmetic surgeries are, they are overshadowed by the continued dominance of the minimally invasive cosmetic procedures, for which 15.9 million were performed last year alone.  Of these, topping the list for another straight year are the Botulinum toxin injectables, the most well-known being Botox@, at 7.4 million cases performed last year, a 3% increase. Falling in at #2 are the soft tissue fillers, such as Juvederm@ and Radiesse@, where 2.6 million procedures were performed at a 2% increase.  Not trailing far behind at #3 are chemical peels at 1.3 million done, a 1% increase from the previous year.  Rounding out the list at #4 and #5 are laser hair removal (1 million cases) and microdermabrasion (709,413 cases), which both show a decline of 1% and 4% respectively.  These ASPS statistics are an accurate reflection of my practice where neurotoxins and soft tissue fillers dominate the injectable market, with Xeomin@ and Radiesse@ being my most popular products.  Also, chemical peels have largely replaced microdermabrasion in my practice, likely due to the ease of application with superior results in my opinion.

Looking Tired? Eyelid Rejuvenation in Irvine, CA May Be for You!

Have you ever been told that you look tired, even though you may feel fine? Often, the first area of our bodies people look at to communicate and ascertain our moods is our eyes, and when the usual sequelae of aging occurs, this can be misconstrued as a sign of tiredness and fatigue. Common aging changes of our eyelids include laxity and hooding of the skin, herniation of the fat pads around our eyes, and weakening of the cartilaginous tarsal sling that supports the eyelid structure and tone. Sun damage can also lead to many fine lines and wrinkles with contour irregularities around the eyes, as well as pigmentation changes that can further add to a worn out and exhausted look.

Dermal Fillers and Injectables

Thankfully, there are procedures available that can dramatically rejuvenate your eyelids and give you that fresh appearance you had when you were younger! These procedures range from simple, minimally invasive office procedures to surgical techniques that address more advanced aging changes. Everyone’s eyelids are different, and each procedure can be tailored to treat unique aspects and concerns for each individual.  Hyperactive orbicularis oculi muscles that lead to the ‘crow’s feet’ appearance upon facial animation can be nicely relaxed using commonly available neurotoxins, such as Botox® or Xeomin®. Mild hollowing of the under-eye tear trough region can be augmented and smoothed out in the office using a hyaluronic acid filler product, such as Restylane® or Juvederm®.

Eyelid Surgery

The more static wrinkles and creases that occur from long-term exposure to the harmful ultraviolet rays of the sun, the better they respond to skin resurfacing procedures, such as microdermabrasion, chemical peels, or CO2 lasers. Although the more aggressive resurfacing procedures can require a few days of downtime, the results can be significant. Finally, for more advanced eyelid changes, the blepharoplasty/eyelid lift procedure can dramatically restore the aesthetic contours and tone of your youth. This procedure, which can be performed under local sedation or general anesthesia, can remove loose skin, excise herniated fat pads, and tighten the weakened tarsal support structure to better define your eyelids.

The Asian Blepharoplasty

Occasionally, the eyelid lift procedure can be used to create a more Westernized look by creating a double-crease of the upper eyelid, which is considered aesthetic in many Asian cultures. The “Asian blepharoplasty” addresses the unique anatomy of the Asian upper eyelid, which usually has more fullness, as well as a lower insertion point of the orbital septum, leading to the classic ‘oriental’ look. The creation of the double-crease has become extremely popular in countries such as China, Japan, and South Korea. Being half-Japanese myself and having visited Japan many times, I have seen a significant rise in interest in these cosmetic procedures. Thorough knowledge of anatomy and surgical techniques are critical to achieving an excellent result.

Several incisions can be used for access including the infra mammary fold, around the areola, and axillary approaches. It is essential to note that breast implants, like all prosthetic devices, do not last forever and typically will need to be replaced every 10-15 years. Interestingly, I have seen patients who have kept their same implants in for over 20 years without problems, but this is not recommended.

In our fast-paced society where we are constantly bombarded with television and print advertising, as well as the ever-increasing popularity of reality shows that sensationalize cosmetic surgery, it is no wonder that the public is often confused as to the safety and realistic expectations of these surgical procedures. Just because a doctor has a fancy ad campaign or their own reality show, do not assume that they are qualified to be performing your procedure.

Many of the horror stories people hear are often from unqualified doctors performing surgeries that lead to serious complications or even death. I have personally witnessed practices with family practitioners performing facelifts, ENT doctors performing breast augmentations and tummy tucks, and dermatologists or gynecologists performing a variety of facial and body cosmetic surgeries with little to no training. Many of these doctors take a weekend course or simply observe another physician performing these procedures and begin doing them at their practices.

Don’t Be Fooled by Fancy Sounding Credentials

The American Board of Medical Specialties (ABMS), the main authoritative organization that provides what is considered the ‘gold standard’ in physician certifications, oversees 24 specialty boards to maintain the highest standards of ethics and medical care for its members. The ONLY board recognized by the ABMS in the discipline of plastic and reconstructive surgery is the American Board of Plastic Surgery (ABPS). This is a critical fact to know since many cosmetic doctors claim membership and ‘board certification’ in random organizations with impressive names designed to deliberately confuse and mislead the public to try to add credibility to their questionable qualifications.

Some physicians, including myself, complete a full five-year general surgery residency, in addition to years of surgical fellowship training to further fine-tune their skills. After completion of this training, candidates must go through grueling written and oral board examinations to prove their knowledge, as well as submit cases for peer review by the board members and fellow practicing surgeons. It is only after successful completion of all these requirements that a physician can earn the distinction of board certification. Although some of the other surgical specialty training programs may provide limited exposure to the field of plastic surgery during their rotations, none approach the breadth of exposure and complexity of cases offered in a formal plastic and reconstructive surgery residency.

What Can You Do to Ensure You’re Choosing a Qualified Plastic Surgeon?

If you are not careful, you may find yourself being operated on by a physician with no formal training in plastic surgery who has not met any requirements to prove their knowledge base or competence in performing your surgery.
